COVID Vaccine- Now Available
COVID 19 Vaccination Schedule Available on Paycor
If you are interested in receiving the COVID-19 vaccine, please contact the NEX clinic at 937-651-6951 or email: nexclinic@maryrutan.org. If you have already contacted the clinic to be on the waiting list to receive the vaccine, they will contact you directly to schedule your appointment. Due to global vaccine supply limitations, this initial allotment will not cover all associates and their immediate family members. However, there will be future vaccination dates that will include a Saturday.
Appointments are scheduled by order received at the clinic. However, they are constrained by:
- Shift, location, and NEX’s ability to accommodate associates during their shift per business needs.
A vaccine registration form will need to be filled out in advance. The printable form is included in this email. Please bring the completed form with you to your appointment. If you can’t print it, hard copies are available at the main campus front desk or clinic.

Si ya usted contactó la clínica para estar en la lista de espera y recibir la vacuna, ellos le contactarán directamente para programar su cita. Debido a las limitaciones de suministro de vacunas a nivel mundial, esta asignación inicial no cubrirá a todos los asociados y sus familiares directos. Sin embargo, habrá futuras fechas de vacunación que incluirán un sábado.
Las citas se programan por orden de llegada a la clínica. Sin embargo, están limitadas por:
- Turno, ubicación y la capacidad de NEX para acomodar a los asociados durante su turno por necesidades del negocio.
Un formulario de registro de vacunas tendrá que ser llenado por adelantado. El formulario imprimible se incluye en este correo electrónico. Por favor, traiga el formulario completado a su cita. Si no puede imprimirlo, hay copias impresas disponibles en la recepción del campus principal, o en la clínica.
